New models of care for people who need support

Welcome to our Adult Social Care Strategy page. Here you can sign-up for one or more of the working groups that are developing different models of care.

These are:


Universal Services and Resilient Communities

  • Helping make sure that people can access support from community resources, facilities, services and local people and groups.


Targeted help (including crisis and reablement support)

  • Temporary, focused support to help people maintain or regain their independence to continue living at home.


Ongoing care

  • For people who need long-term or permanent support, sometimes for the whole of their life, in their own home, in a care home or somewhere else.


Hopefully you’re visiting this page because you’re interested in one or more of these areas. Maybe you’re receiving care and support yourself or you have a relative who is. Or perhaps your work gives you a particular insight and interest. You can become as involved as you like for as long as you like.

Following our consultation on the Adult Social Care Strategy, lots of people are already involved in this work – now we want to reach out to others to expand the range and experiences of people included. 

We particularly want to hear from members of Black and Minority Ethnic communities, people with lived experience and people with disabilities who are currently under-represented in the working groups. It’s really important that the new models of care take account of all the different support needs and preferences that people have in a diverse city like Sheffield.
